Психосоциальные и физические потребности во время интенсивных физических нагрузок могут инициировать стрессовую реакцию, активирующую симпато-адреналовую и гипоталамо-гипофизарно-надпочечниковую оси, что приводит к выбросу стрессовых и катаболических гормонов, воспалительных цитокинов и микробных молекул. В кишечнике обитают триллионы микроорганизмов, которые играют фундаментальную роль во многих аспектах биологии человека, включая метаболизм, нейроэндокринную и иммунную функции. Микробиом и его влияние на поведение хозяина, кишечный барьер и иммунитет являются критическим аспектом оси кишка-мозг. Получены данные, показывающие наличие высокой корреляции между физическим и эмоциональным стрессом во время физических упражнений и изменениями состава желудочно-кишечной микробиоты. Модификации состава микробиоты у профессиональных атлетов могут способствовать повышению эффективности тренировок, улучшению спортивных результатов и сокращать период восстановления после интенсивных физических нагрузок.</p></abstract><trans-abstract xml:lang="en"><p>Fatigue, mood disturbances, lack of performance, and gastrointestinal disturbances are common among athletes during training and competition. Psychosocial and physical demands during intense exercise can initiate a stress response that activates the sympathoadrenal and hypothalamic-pituitary-adrenal axes, resulting in the release of stress and catabolic hormones, inflammatory cytokines, and microbial molecules. The gut is home to trillions of microorganisms that play fundamental roles in many aspects of human biology, including metabolism, neuroendocrine, and immune function. The microbiome and its influence on host behavior, the gut barrier, and immunity is a critical aspect of the gut-brain axis. Data have been obtained showing a high correlation between physical and emotional stress during exercise and changes in the composition of the gastrointestinal microbiota. Modifications to the composition of the microbiota in professional athletes can improve training efficiency, improve athletic performance and shorten the recovery period after intense physical exertion.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>спортсмен</kwd><kwd>физические нагрузки</kwd><kwd>стресс</kwd><kwd>микробиота</kwd><kwd>ось кишка-мозг</kwd></kwd-group><kwd-group xml:lang="en"><kwd>athlete</kwd><kwd>physical activity</kwd><kwd>stress</kwd><kwd>microbiota</kwd><kwd>gut-brain axis</kwd></kwd-group><funding-group><funding-statement xml:lang="ru">Работа выполнялась без спонсорской поддержки</funding-statement><funding-statement xml:lang="en">The work was carried out without sponsorship.</funding-statement></funding-group></article-meta></front><body><sec><title>Введение</title><p>Стресс является важной адаптацией, необходимой для гомеостаза, работоспособности и выживания [<xref ref-type="bibr" rid="cit1">1</xref>][<xref ref-type="bibr" rid="cit2">2</xref>]. Реакция на стресс возникает, когда человек сталкивается с эндогенной или экзогенной проблемой, воспринимаемой как неблагоприятная или угрожающая. Это может быть вызвано физическими, физиологическими или психологическими стимулами [<xref ref-type="bibr" rid="cit1">1</xref>]. Интенсивные физические нагрузки (ФН) подразумевают адаптационные процессы, включающие аффективный, физиологический, биохимический и когнитивно-поведенческий ответ в попытке восстановить гомеостаз [<xref ref-type="bibr" rid="cit3">3</xref>][<xref ref-type="bibr" rid="cit4">4</xref>]. Поэтому трудно провести различие между эффектами физического стресса от ФН и эффектами психологического стресса во время физических упражнений [<xref ref-type="bibr" rid="cit5">5</xref>]. Таким образом, как физические, так и психологические факторы во время интенсивных ФН упоминаются в данной статье как «стресс». Примерно 20– 60 % спортсменов страдают от стресса, вызванного чрезмерными ФН и недостаточным восстановлением [<xref ref-type="bibr" rid="cit6">6</xref>]. Распространённость стресса выше в видах спорта на выносливость, таких как плавание, гребля, езда на велосипеде, триатлон и бег на длинные дистанции, где спортсмены тренируются 4–6 часов в день, 6 дней в неделю, в течение нескольких недель без отдыха от интенсивной тренировки [<xref ref-type="bibr" rid="cit7">7</xref>]. Нет единого мнения относительно того, какие симптомы или биомаркеры определяют стресс [<xref ref-type="bibr" rid="cit2">2</xref>]. В то же время в изученной литературе широко распространены общие признаки стресса, которые включают клинические, гормональные показатели и другие симптомы, связанные с усталостью, снижением работоспособности, бессонницей, изменением настроения, аппетита, потерей массы тела, и такие нарушения, как раздражительность, беспокойство, утрата мотивации, плохая концентрация и депрессия, а также воспаление и иммуносупрессия [<xref ref-type="bibr" rid="cit6">6</xref>].</p><p>Проведён исчерпывающий библиографический поиск в электронных базах данных, включая MEDLINE, Web of Science, Google Scholar PubMed, Scopus и eLibrary с учётом статей, опубликованных в 2000–2023 гг.</p><p>Две основные различные, но взаимосвязанные системы, которые влияют на стрессовую реакцию во время ФН: симпато-адреномедуллярная (СAM) и гипоталамо-гипофизарно-надпочечниковая (кортикостероидная) (ГГК) оси [<xref ref-type="bibr" rid="cit2">2</xref>]. Активация этих осей приводит к выбросу катехоламинов норэпинефрина (НЭ) и адреналина и глюкокортикоидов в системный кровоток [<xref ref-type="bibr" rid="cit8">8</xref>]. Стресс во время ФН также активирует вегетативную нервную систему (ВНС) [<xref ref-type="bibr" rid="cit9">9</xref>], которая обеспечивает немедленный ответ на стрессорный стимул через её симпатический и парасимпатический отделы и увеличивает высвобождение нейронами НЭ и других нейротрансмиттеров в тканях органов желудочно-кишечного тракта (ЖКТ) или сердечно-сосудистой системы [<xref ref-type="bibr" rid="cit8">8</xref>]. Двунаправленная связь между ВНС и нервной системой кишки (НСК), оси кишечника и мозга, главным образом реализуется блуждающим нервом, проходящим от ствола мозга через пищеварительный тракт и регулирующим почти все аспекты перемещения химуса через кишку [<xref ref-type="bibr" rid="cit10">10</xref>]. Другими путями связи оси кишки и головного мозга являются: гормоны кишечника [<xref ref-type="bibr" rid="cit11">11</xref>] (g-аминомасляная кислота (ГАМК), нейропептид Y, дофамин) и молекулы микробиоты кишки — короткоцепочечные жирные кислоты (КЖК), триптофан [<xref ref-type="bibr" rid="cit12">12</xref>][<xref ref-type="bibr" rid="cit13">13</xref>].</p><p>В кишечнике человека содержится более 100 триллионов микроорганизмов, которые представляют в ЖКТ примерно 9 миллионов генов [<xref ref-type="bibr" rid="cit14">14</xref>][<xref ref-type="bibr" rid="cit15">15</xref>]. Кишечная микробиота толстой кишки состоит из пяти типов и около 160 видов [<xref ref-type="bibr" rid="cit16">16</xref>]. Микробиота способствует пищеварению и всасыванию пищи для производства энергии хозяина и обеспечивает фолиевой кислотой, витамином К2 и КЖК [17–20]. В толстой кишке человека сложные углеводы перевариваются и затем ферментируются анаэробной кишечной микробиотой в КЖК, N-бутират, ацетат и пропионат [<xref ref-type="bibr" rid="cit17">17</xref>]. Микробиота нейтрализует лекарства, наркотики и канцерогены, модулирует моторику кишки, защищает хозяина от патогенов, способствует созреванию иммунной системы и дифференцировке эпителиальных клеток слизистой оболочки ЖКТ [<xref ref-type="bibr" rid="cit21">21</xref>]. Фактические данные показывают, что кишечная микробиота модулирует возбуждающие и ингибирующие нейротрансмиттеры (серотонин, ГАМК, дофамин) и вещества, подобные нейромедиаторам, особенно в ответ на физическое и эмоциональное напряжение (стресс) [<xref ref-type="bibr" rid="cit2">2</xref>][<xref ref-type="bibr" rid="cit22">22</xref>]. Систематический обзор ФН на выносливость и микробиоту кишки [<xref ref-type="bibr" rid="cit20">20</xref>][<xref ref-type="bibr" rid="cit23">23</xref>] предполагает, что последняя может играть ключевую роль в контроле окислительного стресса и воспалительных реакций, а также в улучшении обмена веществ и расхода энергии во время интенсивных ФН [<xref ref-type="bibr" rid="cit19">19</xref>][<xref ref-type="bibr" rid="cit24">24</xref>]. Однако, помимо этих функций, связь между стрессом, индуцированным ФН, и составом кишечной микробиоты, а также участие возможных патофизиологических механизмов до конца не изучены [<xref ref-type="bibr" rid="cit2">2</xref>][<xref ref-type="bibr" rid="cit25">25</xref>]. Корреляция между физической работоспособностью спортсменов и микробиотой их кишечника в последние годы вызывает растущий интерес, поскольку появились новые данные о важности микробиоты как основного фактора, определяющего состояние здоровья профессиональных спортсменов и их возможности в достижении высоких результатов на соревнованиях [<xref ref-type="bibr" rid="cit20">20</xref>][26–28].</p><p>Цель настоящего аналитического обзора состоит в обобщении имеющихся данных о взаимодействии между кишечной микробиотой и осью кишка-мозг при стрессе, вызванном ФН, и анализе механизмов, в которых микробиота может играть положительную роль, влияющую на спортивные результаты, а также сокращать время восстановления во время тренировок у элитных спортсменов.</p></sec><sec><title>Роль микробиоты в контроле выброса гормонов, связанных со стрессом при физической нагрузке</title><p>Профессиональные спортсмены, тренирующиеся и соревнующиеся в течение нескольких часов, испытывают физический и эмоциональный стресс, вызывающий сдвиги в физиологическом гомеостазе, стимулирующем оси СAM и ГГК [<xref ref-type="bibr" rid="cit3">3</xref>]. Система СAM, являющаяся частью симпатического отдела ВНС, высвобождает адреналин из мозгового вещества надпочечников, что способствует быстрой мобилизации метаболических ресурсов и регуляции ответа борьба/бегство [<xref ref-type="bibr" rid="cit8">8</xref>]. Как правило, это повышает уровень циркулирующего адреналина в крови (в основном из мозгового вещества надпочечников) и норадреналина (в первую очередь из симпатических нервов), частоту и силу сердечных сокращений, периферическую вазоконстрикцию и мобилизацию энергии. Парасимпатический тонус также может модулироваться во время стресса [<xref ref-type="bibr" rid="cit2">2</xref>][<xref ref-type="bibr" rid="cit8">8</xref>].</p><p>С другой стороны, стрессовые стимулы активируют нейросекреторные мелкоклеточные ядра гипоталамуса, продуцирующие гипофизотропные факторы (кортикотропин-рилизинг-гормон (КТРГ)) [<xref ref-type="bibr" rid="cit2">2</xref>][<xref ref-type="bibr" rid="cit8">8</xref>]. Аксоны нейросекреторных клеток этих ядер образуют терминали на первичной капиллярной сети в срединном возвышении (нейрогемальной контактной зоне). Эта сеть собирается в воротные вены, проникающие в переднюю долю гипофиза и распадающиеся на вторичную сеть капилляров между тяжами аденоцитов, где стимулируют выброс кортикотропами адренокортикотропного гормона (АКТГ) в системное кровообращение [<xref ref-type="bibr" rid="cit8">8</xref>]. АКТГ взаимодействует с рецепторами коры надпочечников, стимулируя выработку и выброс глюкокортикоидов (ГК) в общую циркуляцию. Эффект ГК зависит от рецепторов, с которыми они связываются. Выделяют два рецептора ГК: рецептор минералокортикоидов (РМ) и рецептор глюкокортикоидов (РГ). Вне головного мозга ГК работают через РГ, тогда как в мозге ГК связываются как с РМ, так и с РГ [<xref ref-type="bibr" rid="cit8">8</xref>]. РГ опосредуют большинство стрессовых эффектов глюкокортикоидов (включая метаболизм и иммунитет). Связываясь с РГ, ГК ингибируют дальнейшее высвобождение КТРГ, регулируя тем самым секреторную активность клеток гиппокампа, и опосредуют большинство основных эффектов, которые включают поддержание чувствительности нейронов к своим нейротрансмиттерам, циркадного ритма ГГК и артериального давления [<xref ref-type="bibr" rid="cit29">29</xref>].</p><p>Острая ФН выше 60 % максимального содержания кислорода (VO2 max) является одним из физических стрессов, которые стимулируют ось ГГК и высвобождение катаболических гормонов [<xref ref-type="bibr" rid="cit30">30</xref>], в то время как ФН ниже этой интенсивности не вызывают такой скачок уровня кортизола в сыворотке [<xref ref-type="bibr" rid="cit31">31</xref>]. ФН с нагрузкой 80 % VO2 max провоцируют значительное повышение уровня АКТГ до и после тренировки [<xref ref-type="bibr" rid="cit31">31</xref>]. Комплексные исследования выносливости [<xref ref-type="bibr" rid="cit2">2</xref>] в течение последних 20 лет показали, что 60–80 % спортсменов на ранней стадии хронического стресса имеют более высокий ответ АКТГ гипофизом, стимулируемый КТРГ. Следовательно, существует чёткая связь между стрессом, вызванным ФН, и повышением уровня гормона стресса у спортсменов.</p><p>Стресс во время ФН также активирует ВНС [<xref ref-type="bibr" rid="cit9">9</xref>], которая увеличивает высвобождение нейронами НЭ и других нейротрансмиттеров в периферических тканях, в том числе ЖКТ. ФН и морфофункциональный гомеостаз ЖКТ связаны между собой [<xref ref-type="bibr" rid="cit4">4</xref>][<xref ref-type="bibr" rid="cit32">32</xref>]. Двунаправленная связь между ВНС и НСК в ЖКТ, оси кишки и головного мозга, в основном реализуется через блуждающий нерв, который проходит от ствола мозга через пищеварительный тракт [<xref ref-type="bibr" rid="cit33">33</xref>]. Помимо нейронной связи, другие пути коммуникации оси кишка-мозг осуществляются через гормоны эндокринных энтероцитов [<xref ref-type="bibr" rid="cit11">11</xref>] и молекулы кишечной микробиоты [<xref ref-type="bibr" rid="cit12">12</xref>][<xref ref-type="bibr" rid="cit34">34</xref>].</p><p>Появляется всё больше доказательств того, что эндокринные клетки ЖКТ реагируют на стресс путём высвобождения гормонов, таких как ГАМК, нейропептид Y (NPY) и дофамин [<xref ref-type="bibr" rid="cit35">35</xref>]. ГАМК является основным ингибирующим нейромедиатором ЦНС, регулирует артериальное давление, частоту сердечных сокращений, функции ЖКТ (моторика, опорожнение желудка, временное расслабление нижнего сфинктера пищевода), а также играет важную роль в развитии тревоги, депрессии, болевых ощущений и иммунного ответа [<xref ref-type="bibr" rid="cit36">36</xref>]. Умеренные ФН могут повысить уровень ГАМК в гипоталамусе, что приводит к снижению эффективности отдыха, артериального давления, частоты сердечных сокращений и симпатического тонуса [<xref ref-type="bibr" rid="cit37">37</xref>].</p><p>NPY высвобождается эндокриноцитами слизистой оболочки ЖКТ в ответ на стрессовые стимулы, такие как интенсивные ФН, и ослабляет ось ГГК [<xref ref-type="bibr" rid="cit38">38</xref>]. NPY представляет собой пептид из 36 аминокислот, локализующийся по всей оси кишка-мозг; является наиболее распространённым нейропептидом в головном мозге, играющим роль в устойчивости к стрессу и воспалительным процессам [<xref ref-type="bibr" rid="cit39">39</xref>]. При изучении уровня NPY в сыворотке и гиппокампе у 12 высококвалифицированных гребцов обнаружено [<xref ref-type="bibr" rid="cit38">38</xref>], что концентрации NPY после тренировки значительно увеличивались. Эти результаты свидетельствуют, что нейропептид Y играет роль в снижении стрессовой реакции при интенсивных ФН.</p><p>Дофамин, предшественник НЭ и адреналина, также синтезируется во время стресса в ЖКТ. Производство дофамина зависит от нескольких факторов: уровня его предшественника тирозина, кишечных бактерий, которые непосредственно производят дофамин, типа переживаемого стресса и пола [<xref ref-type="bibr" rid="cit40">40</xref>]. На протяжении кишки имеется несколько дофаминовых рецепторов, что позволяет обсуждать его роль в оси кишка-мозг [<xref ref-type="bibr" rid="cit40">40</xref>]. ЖКТ, селезёнка и поджелудочная железа вырабатывают значительное количество дофамина [<xref ref-type="bibr" rid="cit40">40</xref>]. Фермент, ограничивающий скорость синтеза дофамина, тирозингидроксилаза, обнаружен в желудочных эндокриноцитах человека, что показывает, что его функция реализуется за пределами нейротрансмиссии головного мозга [<xref ref-type="bibr" rid="cit40">40</xref>]. Привычная ФН в течение 1–2 часов в день повышает уровень дофамина в головном мозге [<xref ref-type="bibr" rid="cit41">41</xref>].</p><p>Исследования оси кишка-мозг обращают внимание на участие микробиоты и её молекул в управлении реакцией тревоги и депрессии [<xref ref-type="bibr" rid="cit42">42</xref>]. Однако роль микробиоты в контроле адаптации к стрессу, вызванному ФН, остаётся неизвестной. Использование стерильных животных без микробов даёт представление об участии микробиоты в регуляции развития и функции оси ГГК в ответ на стресс [<xref ref-type="bibr" rid="cit2">2</xref>]. Установлено, что микробный состав кишки имеет решающее значение для развития и реализации соответствующей реакции на стресс и оси ГГК [<xref ref-type="bibr" rid="cit43">43</xref>]. Кроме того, появляется всё больше экспериментальных свидетельств того, что комменсальное и резидентное сообщество кишечных микроорганизмов может регулировать ось ГГК посредством синтеза гормонов и нейротрансмиттеров, таких как ГАМК, дофамин и серотонин [<xref ref-type="bibr" rid="cit2">2</xref>]. Более того, экспериментальные исследования показывают, что блуждающий нерв служит своего рода «горячей линией», по которой кишечные микробы напрямую связываются с ЦНС [<xref ref-type="bibr" rid="cit10">10</xref>]. Учитывая очевидную связь между событиями в раннем онтогенезе и последующим ответом нейрогенеза взрослых животных на стресс [<xref ref-type="bibr" rid="cit44">44</xref>], необходимо выяснить, могут ли потенциальные последствия нарушений микробиоты в детском возрасте влиять на нейробиологию стресса и эндокринную функцию микробиоты. До сих пор не хватает убедительных доказательств того, что кишечная микробиота является причиной стресса [<xref ref-type="bibr" rid="cit2">2</xref>][<xref ref-type="bibr" rid="cit10">10</xref>].</p><p>Растёт интерес к тому, как кишечная микробиота напрямую взаимодействует с гормонами стресса в периферических тканях слизистой оболочки ЖКТ, это получило обозначение как микробная эндокринология [<xref ref-type="bibr" rid="cit12">12</xref>]. Показано прямое влияние НЭ на Aeromonas hydrophila, виды Bordetella, Campylobacter jejuni, Helicobacter pylori, виды Listeria и Salmonella enterica spp., на кишечник. Механизмы, которыми НЭ способствует росту патогенных бактерий, заключаются в обеспечении прикрепления E. coli к стенке кишки путём увеличения экспрессии его фактора вирулентности K99 pilus adhesin, а также активации экспрессии связанных с вирулентностью факторов в Salmonella typhimurium, облегчающих заражение этими бактериями [<xref ref-type="bibr" rid="cit12">12</xref>]. Кроме того, показано, что НЭ повышает уровни непатогенных E. coli и других грамотрицательных бактерий [<xref ref-type="bibr" rid="cit45">45</xref>].</p><p>Только одно исследование показало, что стресс, вызванный ФН, напрямую изменяет состав кишечной микробиоты у стерильных или не содержащих специфических антигенов животных (SPF). Опубликованы материалы, расширяющие понимание того, как микробиом регулирует стрессовую реакцию, вызванную ФН, выявляя взаимодействия микробиота-хозяин, которые важны для поддержания структурно-функционального гомеостаза ЖКТ и системного здоровья [<xref ref-type="bibr" rid="cit2">2</xref>][<xref ref-type="bibr" rid="cit19">19</xref>].</p></sec><sec><title>Роль микробиоты в контроле симптомов желудочно-кишечных нарушений, связанных со стрессом при физической нагрузке</title><p>Известно более 50 белков, играющих важную роль в регуляции плотных соединений эндотелиального монослоя сосудов слизистой оболочки ЖКТ и, следовательно, кишечной проницаемости [<xref ref-type="bibr" rid="cit46">46</xref>]. Плотные контакты состоят из 4 трансмембранных белков: окклюдина, клаудинов, соединительных молекул адгезии и трицеллюлина, которые взаимодействуют со структурными белками zonula occludens (ZO1, ZO2 и ZO3) [<xref ref-type="bibr" rid="cit46">46</xref>]. В нормальных условиях комплексы плотных соединений обеспечивают поддержание поляризации кишечного барьера, контролирующего межклеточное прохождение только небольших молекул, ионов, воды и лейкоцитов [<xref ref-type="bibr" rid="cit46">46</xref>]. Кишечный барьер также является «дверью» между микроорганизмами и их побочными продуктами, определяет реакцию кишечно-ассоциированной лимфоидной ткани иммунной системы на антигенные раздражители внутри и вне ЖКТ [<xref ref-type="bibr" rid="cit47">47</xref>]. Согласно данным, основанным на оценке острого влияния ФН на иммунные и воспалительные реакции у нетренированных взрослых [<xref ref-type="bibr" rid="cit48">48</xref>], повышенная проницаемость кишки («leaky gut — дырявый, протекающий кишечник») представляет собой деструкцию белковых мембранных структур плотных контактов. Чрезмерное выделение гормонов, вызванное физическим и психологическим стрессом, может вызвать транслокацию липополисахаридов (ЛПС) за пределы ЖКТ с иммунными и воспалительными реакциями, часто приводящими к повышенной проницаемости кишки [<xref ref-type="bibr" rid="cit48">48</xref>]. Транслоцированные ЛПС обнаруживается CD14 и toll-подобным рецептором 4 (TLR4), которые вызывают высвобождение провоспалительных цитокинов: фактора некроза опухоли альфа (TNF-α), интерферона альфа (IFN-α), интерферона гамма (INF-γ) и интерлейкинов (IL-1β или IL-6), что в конечном итоге приводит к эндотоксемии [<xref ref-type="bibr" rid="cit49">49</xref>]. Провоспалительные цитокины также увеличивают открытие плотных соединений через ZO1 и ZO2, инициирующие эндотоксемию [<xref ref-type="bibr" rid="cit49">49</xref>]. Кроме того, активация оси ГГК может стимулировать секрецию субэпителиальными тучными клетками иммуномедиаторов (гистамин, протеазы и провоспалительные цитокины [<xref ref-type="bibr" rid="cit50">50</xref>]), повышающих кишечную проницаемость [<xref ref-type="bibr" rid="cit51">51</xref>].</p><p>В зависимости от типа ФН, возраста и других факторов, у 20–50 % спортсменов возникают симптомы желудочно-кишечных нарушений, которые усиливаются с увеличением ФН [<xref ref-type="bibr" rid="cit4">4</xref>][<xref ref-type="bibr" rid="cit52">52</xref>]. Гипертермия, ишемия и гипоперфузия являются стимулами, вызывающими деструкцию плотных контактов во время значительных ФН [<xref ref-type="bibr" rid="cit2">2</xref>]. Это обычное явление у спортсменов во время интенсивных ФН, когда температура тела повышается и кровь перераспределяется от ЖКТ к соматическим мышцам, сердцу и лёгким [<xref ref-type="bibr" rid="cit49">49</xref>]. Перекись водорода служит сигнальной молекулой, активирующей транскрипцию нескольких провоспалительных генов, включая ядерный фактор, каппа-энхансер-активатор активированных В-клеток (NF-kβ), TNF-α, IL-6, IFN-γ и IL-1β, компрометирующих барьерную функцию [<xref ref-type="bibr" rid="cit2">2</xref>]. Поэтому гипоперфузия и ишемия приводят к увеличению проницаемости кишки, открывая путь для циркуляции в кровотоке ЛПС и кишечных бактерий, что вероятно инициирует эндотоксемию [<xref ref-type="bibr" rid="cit53">53</xref>][<xref ref-type="bibr" rid="cit54">54</xref>]. У спортсменов с ФН на ультравыносливость (марафонцы, триатлонисты) концентрация ЛПС в плазме составляет от 5 до 284 пг/мл, а до 93 % обследованных сообщают о нарушениях пищеварения, которые могли быть вызваны ЛПС-цитокиновой реакцией [<xref ref-type="bibr" rid="cit2">2</xref>]. При этом у 81 % случайно отобранных истощённых марафонцев обнаружили эндотоксемию (0,1 нг/мл), у 2 % — смертельные уровни выше 1 нг/мл, и только у 19 % были нормальные уровни этого показателя [<xref ref-type="bibr" rid="cit55">55</xref>]. Кроме того, 81 % бегунов с высоким уровнем ЛПС также страдали от расстройств ЖКТ, таких как тошнота, диарея и/или рвота, в то время как только 18 % атлетов с низкой концентрацией эндотоксина в плазме сообщили о подобных симптомах. Участники марафона, затратившие более 8 часов для прохождения гонки, имели более высокие концентрации эндотоксина в плазме [<xref ref-type="bibr" rid="cit55">55</xref>]. Установлено, что ГК, высвобождаемые во время интенсивных ФН, снижают экспрессию рецептора TLR и способность продуцировать противовоспалительные цитокины и обеспечивать противомикробную защиту хозяина [<xref ref-type="bibr" rid="cit56">56</xref>]. Эти исследования показывают, что интенсивные ФН в течение длительных периодов приводят к увеличению кишечной проницаемости и повышению уровня ЛПС в плазме, но также вызывают иммуносупрессию [<xref ref-type="bibr" rid="cit2">2</xref>].</p><p>Учитывая разнообразную роль микрофлоры кишки в функции ЖКТ, местном иммунитете ЖКТ [<xref ref-type="bibr" rid="cit57">57</xref>], микробной эндокринологии [<xref ref-type="bibr" rid="cit12">12</xref>][<xref ref-type="bibr" rid="cit13">13</xref>], а также регуляции окислительного стресса [<xref ref-type="bibr" rid="cit2">2</xref>] и уровня гидратации, неудивительно, что в настоящее время значительно повышаются усилия исследователей идентифицировать механизмы, посредством которых микробиота сможет улучшить кишечный барьер элитных и профессиональных спортсменов [<xref ref-type="bibr" rid="cit20">20</xref>][<xref ref-type="bibr" rid="cit24">24</xref>].</p><p>В толстой и слепой кишке сложные полисахариды растительного происхождения перевариваются и затем ферментируются микроорганизмами Lactobacillus, Bifidobacterium, Clostridium, Bacteroides в КЖК и газы, которые используются в качестве источников углерода и энергии специализированными бактериями (восстановительные ацетогены, сульфатредуцирующие бактерии и метаногены) [<xref ref-type="bibr" rid="cit20">20</xref>][<xref ref-type="bibr" rid="cit24">24</xref>][<xref ref-type="bibr" rid="cit27">27</xref>][<xref ref-type="bibr" rid="cit34">34</xref>]. Состав микробиоты, метаболические взаимодействия между видами микроорганизмов, а также количество и тип основных пищевых макро- и микроэлементов определяют типы и количество КЖК, продуцируемых кишечными микроорганизмами [<xref ref-type="bibr" rid="cit25">25</xref>][<xref ref-type="bibr" rid="cit58">58</xref>]. Чем больше полисахаридов растительного происхождения, олигосахаридов, устойчивого крахмала и клетчатки получает человек, тем интенсивнее его бактерии ферментируют неперевариваемые компоненты пищи в полезный КЖК. Продуцируемые микробиотой КЖК влияют на ряд процессов хозяина, включая контроль рН толстой кишки, с последующим воздействием на состав микробиоты, подвижность и проницаемость кишки, пролиферацию эпителиоцитов слизистой оболочки [<xref ref-type="bibr" rid="cit20">20</xref>][<xref ref-type="bibr" rid="cit59">59</xref>]. N-бутират, вырабатываемый кишечными бактериями, регулирует функцию и миграцию нейтрофилов, ингибирует индуцированную воспалительными цитокинами экспрессию молекулы-1 адгезии клеток сосудов, повышает экспрессию белков плотных контактов энтероцитов толстой кишки и обладает противовоспалительным эффектом [<xref ref-type="bibr" rid="cit21">21</xref>]. N-бутират и пропионат повышают трансэпителиальную резистентность, улучшающую барьерную функцию кишки, и уменьшают воспаление [<xref ref-type="bibr" rid="cit60">60</xref>], служат основным источником энергии для 60–70 % колоноцитов [<xref ref-type="bibr" rid="cit61">61</xref>], что предотвращает деградацию слизистой оболочки [<xref ref-type="bibr" rid="cit62">62</xref>], которая возникает, например, в результате интенсивных тренировок из-за гипоперфузии и ишемии. Показано, что в ответ на ФН в слепой кишке происходят значительные морфофункциональные сдвиги, изменяются профили микробиоты и КЖК [<xref ref-type="bibr" rid="cit20">20</xref>][<xref ref-type="bibr" rid="cit63">63</xref>].</p><p>Стресс, вызванный ФН, деградирует барьерную функцию кишки и вызывает транслокацию ЛПС, приводящую к нарушениям ЖКТ, гидратации, сдвигу усвоения питательных веществ и электролитов, а также к термическому повреждению слизистой оболочки, что негативно влияет на спортивные показатели [<xref ref-type="bibr" rid="cit2">2</xref>][<xref ref-type="bibr" rid="cit54">54</xref>]. В то же время данные исследований уровня КЖК и энергетического обмена в слепой кишке показывают, как с помощью интенсивных ФН можно влиять на выработку КЖК и воздействовать на ось ГГК и структурно-функциональный гомеостаз ЖКТ с целью улучшения спортивных достижений [<xref ref-type="bibr" rid="cit20">20</xref>][<xref ref-type="bibr" rid="cit24">24</xref>].</p></sec><sec><title>Заключение</title><p>Стресс, вызванный ФН, влияет на катаболические гормоны, цитокины и микробные молекулы кишечника, что приводит к желудочно-кишечным расстройствам и снижению спортивных достижений. Микробиота кишки играет фундаментальную роль во многих аспектах биологии человека, включая метаболизм, нейроэндокринную и иммунную функции. Интенсивный стресс, вызванный ФН, усугубляет воспаление кишечника и клинические исходы за счёт снижения видов Turicibacter spp. и увеличения количества Ruminococcus gnavus, Butyrivibrio spp., Oscillospira spp. и Coprococcus spp. В свете этих предварительных результатов, изменения функции ЖКТ спортсменов отражают основное взаимодействие между кишечной микробиотой и осью кишка-мозг во время интенсивных ФН.
