Несмотря на высокую распространённость, ЛИУВТ остаётся вне поля должного клинического внимания, что способствует снижению приверженности лечению, ухудшению кардиометаболических показателей и снижению качества жизни пациентов.</p></sec><sec><title>Цель обзора</title><p>Цель обзора. В настоящей статье представлен всесторонний анализ патофизиологических механизмов, лежащих в основе ЛИУВТ, включая модуляцию центральных нейромедиаторных систем (серотонин, гистамин, дофамин), эндокринные нарушения (гиперинсулинемия, лептинорезистентность), изменение энергетического обмена, а также новые данные о роли кишечной микробиоты и генетической предрасположенности.</p><p>Основные классы препаратов. Подробно рассмотрены ключевые классы лекарственных средств, ассоциированные с увеличением массы тела: антипсихотики (особенно атипичные), антидепрессанты, антигипертензивные препараты, глюкокортикостероиды и гипогликемические средства (инсулин, производные сульфонилмочевины). Для каждого класса приведены данные о степени прибавки массы тела, механизмах развития и метаболических последствиях.</p></sec><sec><title>Стратегии ведения</title><p>Стратегии ведения. Представлены современные подходы к профилактике и терапии ЛИУВТ, включающие выбор препаратов с благоприятным метаболическим профилем (метформин, топирамат, агонисты рецепторов глюкагоноподобного пептида-1 (ГПП-1), ингибиторы натрий-глюкозного котранспортера 2-го типа (SGLT2)), применение адъювантной терапии и немедикаментозные интервенции. Подчёркнута важность раннего скрининга (индекс массы тела, окружность талии, уровни глюкозы и липидов) и мультидисциплинарного подхода с участием психиатров, эндокринологов и кардиологов.</p></sec><sec><title>Заключение</title><p>Заключение. Персонализированная стратегия, сочетающая терапевтическую эффективность с метаболической безопасностью, является основой для минимизации ятрогенных рисков и улучшения долгосрочных исходов лечения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Drug-induced weight gain (DIWG) is a common and often overlooked adverse effect of many widely prescribed medications, representing a significant interdisciplinary challenge. Despite its high prevalence, DIWG remains underrecognized in clinical practice, contributing to poor treatment adherence, worsening of cardiometabolic risk factors, and diminished quality of life. </p></sec><sec><title>Objective</title><p>Objective. This review provides a comprehensive analysis of the pathophysiological mechanisms underlying medication-associated weight gain, including modulation of central neurotransmitter systems (serotonin, histamine, dopamine), endocrine alterations (hyperinsulinemia, leptin resistance), changes in energy expenditure, as well as the emerging roles of gut microbiota and genetic susceptibility. </p></sec><sec><title>Key drug classes</title><p>Key drug classes. The article details the major drug classes implicated in weight gain: antipsychotics (particularly atypical agents), antidepressants, antihypertensives, corticosteroids, and antihyperglycemic drugs (insulin, sulfonylureas). For each class, the magnitude of weight change, underlying mechanisms, and metabolic consequences are discussed. </p></sec><sec><title>Management strategies</title><p>Management strategies. Current approaches to the prevention and treatment of DIWG are presented, emphasizing the selection of medications with a favorable metabolic profile (e. g., metformin, topiramate, glucagon-like peptide-1 receptor agonists, sodium-glucose cotransporter-2 inhibitors), the use of adjunctive therapies, and non-pharmacological interventions. Согласно данным Всемирной организации здравоохранения, в 2016 г. более 1,9 млрд взрослых имели избыточную массу тела, из них свыше 650 млн страдали ожирением [<xref ref-type="bibr" rid="cit1">1</xref>]. В Российской Федерации доля лиц с избыточной массой тела составила 62,0%, с ожирением — 26,2% [<xref ref-type="bibr" rid="cit1">1</xref>]. По прогнозам, к 2030 г. до 60% населения мира могут иметь избыточную массу тела или ожирение, если текущие тенденции сохранятся [<xref ref-type="bibr" rid="cit1">1</xref>]. Распространённость ожирения среди мужчин в РФ достигает 30%, среди женщин — 39,5%, при этом абдоминальное ожирение регистрируется у 30,9% мужчин и 55,1% женщин [<xref ref-type="bibr" rid="cit2">2</xref>].</p><p>Наряду с общеизвестными факторами (гиподинамия, гиперкалорийное питание, генетическая предрасположенность), существенный вклад в рост распространённости избыточной массы тела вносит ятрогенный фактор. Согласно данным метаанализов, до 20–25% случаев впервые выявленного ожирения у взрослых могут быть ассоциированы с приёмом лекарственных средств, назначаемых по поводу хронических заболеваний [3, 4]. В США около 50% взрослых принимают хотя бы один лекарственный препарат в течение 30 дней, и многие из них обладают потенциалом по увеличению массы тела [<xref ref-type="bibr" rid="cit5">5</xref>].</p><p>Лекарственно-индуцированное увеличение массы тела (ЛИУВТ) часто рассматривается как «тихий» побочный эффект, который либо недооценивается клиницистами, либо приносится в жертву ради контроля основного заболевания (например, купирования психотической симптоматики или достижения целевых уровней глюкозы). Между тем, увеличение массы тела на фоне фармакотерапии ведёт к ухудшению приверженности, повышению риска сердечно-сосудистых осложнений, прогрессированию стеатоза печени и снижению качества жизни пациентов [<xref ref-type="bibr" rid="cit5">5</xref>]. В клинических рекомендациях Минздрава России (2024 г.) ятрогенное ожирение выделено в отдельную рубрику (код Е66.1 по МКБ-10) [<xref ref-type="bibr" rid="cit1">1</xref>], что подчёркивает актуальность данной проблемы.</p><p>Патофизиологические механизмы</p><p>Механизмы ЛИУВТ являются сложными и мультифакторными. Ключевыми патогенетическими звеньями выступают [6, 7]:</p><p>Основные классы лекарственных средств, индуцирующих набор массы тела</p><p>Антипсихотические средства. Наиболее изученной группой являются атипичные антипсихотики (второго поколения). Риск увеличения массы тела варьирует в зависимости от препарата: наиболее высокий — для клозапина и оланзапина (прибавка 4–10 кг за первые 6–12 месяцев, у 80% пациентов прибавка превышает 20% от исходной массы тела) [8, 25]; средний — для рисперидона и кветиапина (прибавка 2–4 кг) [<xref ref-type="bibr" rid="cit26">26</xref>]; минимальный — для арипипразола, зипрасидона, амисульприда, азенапина, илоперидона, луразидона, палиперидона, которые ассоциированы с наименьшим метаболическим риском [27, 28].</p><p>Патогенез: мощный антагонизм H1-рецепторов и 5-HT2C-рецепторов, а также влияние на AMP-активируемую протеинкиназу в адипоцитах. Важно отметить, что у пациентов, впервые получающих антипсихотики, прибавка массы тела может быть более выраженной, особенно у детей и подростков [<xref ref-type="bibr" rid="cit29">29</xref>]. Прирост массы тела в первый месяц лечения является сильным предиктором долгосрочного набора массы тела, что позволяет своевременно принять меры [<xref ref-type="bibr" rid="cit30">30</xref>].</p><p>Кроме того, антипсихотики ассоциированы с развитием сахарного диабета 2 типа (СД2) и метаболического синдрома даже без значительного увеличения массы тела, что указывает на прямое влияние на β-клетки поджелудочной железы и периферическую чувствительность к инсулину [31, 32]. Частота метаболического синдрома у пациентов с шизофренией достигает 32%, а риск СД2 в 2–3 раза выше, чем в общей популяции [<xref ref-type="bibr" rid="cit33">33</xref>].</p><p>Антидепрессанты и стабилизаторы настроения. Трициклические антидепрессанты (амитриптилин, нортриптилин) и ингибиторы моноаминоксидазы ассоциированы со значительной прибавкой массы тела — в среднем 2–4 кг, а у отдельных пациентов до 9 кг [34, 35]. Среди селективных ингибиторов обратного захвата серотонина (СИОЗС) наиболее неблагоприятен пароксетин (прибавка 1,7–3,6 кг); циталопрам также связан с прибавкой, тогда как флуоксетин и сертралин в долгосрочной перспективе относительно нейтральны [36, 37]. Бупропион, напротив, последовательно демонстрирует потерю массы тела (0,4–2,4 кг) и может рассматриваться как метаболически благоприятная альтернатива, особенно у пациентов с ожирением [38, 39]. В комбинации с налтрексоном бупропион зарегистрирован как препарат для лечения ожирения [<xref ref-type="bibr" rid="cit40">40</xref>].</p><p>Литий и вальпроевая кислота также способствуют набору массы тела. У 60% пациентов, принимающих литий, наблюдается прибавка более 5% от исходной массы тела [<xref ref-type="bibr" rid="cit41">41</xref>]. Для вальпроата характерно развитие инсулинорезистентности и гиперинсулинемии, что связано с прямым действием на β-клетки поджелудочной железы (GABA-опосредованное усиление секреции инсулина), повышением уровня лептина и снижением адипонектина [14, 42]. У 60% пациентов, получающих вальпроат, развивается неалкогольная жировая болезнь печени [<xref ref-type="bibr" rid="cit43">43</xref>].</p><p>Гипогликемические средства. Парадоксально, но препараты, призванные контролировать диабет, часто усугубляют ожирение. Инсулин вызывает дозозависимую прибавку массы тела за счёт нескольких механизмов: стимуляции аппетита, особенно при гипогликемических эпизодах, снижения глюкозурии и анаболического действия (увеличение мышечной и жировой массы) [44, 45]. Прибавка массы тела на инсулине может составлять от 0,4 до 4,8 кг, причём более выражена при использовании быстродействующих инсулинов по сравнению с базальными [<xref ref-type="bibr" rid="cit46">46</xref>]. Инсулин детемир обладает относительным эффектом по снижению массы тела, механизм которого до конца не ясен [<xref ref-type="bibr" rid="cit47">47</xref>].</p><p>Производные сульфонилмочевины (глибенкламид, глимепирид) также вызывают гиперинсулинемию и прибавку массы тела: в исследованиях UKPDS и ADOPT прирост составил около 4 кг в первый год лечения [48, 49]. Гликлазид и глимепирид ассоциированы с меньшей прибавкой массы тела, чем более старые препараты сульфонилмочевины [<xref ref-type="bibr" rid="cit50">50</xref>].</p><p>Тиазолидиндионы (пиоглитазон, росиглитазон) вызывают увеличение подкожной жировой клетчатки за счёт дифференцировки преадипоцитов, но при этом могут снижать содержание висцерального жира и улучшать стеатоз печени [51, 52]. Прибавка массы тела составляет 1,5–4 кг в первый год.</p><p>В отличие от этих классов, метформин ассоциирован с потерей массы тела (1–2,9 кг) [3, 53]. Ингибиторы дипептидилпептидазы-4 (ДПП-4) (ситаглиптин, саксаглиптин, линаглиптин) являются нейтральными или вызывают незначительную потерю массы тела [<xref ref-type="bibr" rid="cit54">54</xref>]. Агонисты рецепторов ГПП-1 (эксенатид, лираглутид, семаглутид) и ингибиторы SGLT2 (канаглифлозин, дапаглифлозин, эмпаглифлозин) обеспечивают устойчивую потерю массы тела, что делает их предпочтительными у пациентов с ожирением и СД2 [3, 55, 56].</p><p>Глюкокортикостероиды. Системное (и в меньшей степени ингаляционное) применение глюкокортикоидов ведёт к развитию ятрогенного синдрома Кушинга. По данным популяционных исследований, у 70% пациентов, получающих длительную терапию глюкокортикоидами, наблюдается значительная прибавка массы тела, а у 20% — увеличение массы тела более чем на 10 кг в первый год лечения [57, 58]. Преднизолон ассоциирован с прибавкой 1,7–5,8 кг, преднизолон — 1,5–4,4 кг, кортизон — 1,5–8,4 кг [59, 60].</p><p>Механизм включает стимуляцию аппетита за счёт изменения активности AMP-активируемой протеинкиназы в гипоталамусе, повышение предпочтения жирной пищи, активацию эндоканнабиноидной системы в печени и жировой ткани (через каннабиноидный рецептор 1 типа), а также индукцию инсулинорезистентности [11, 61, 62]. Важно отметить, что даже низкие дозы преднизолона (5–10 мг/сут) могут вызывать значимые метаболические нарушения [<xref ref-type="bibr" rid="cit63">63</xref>].</p><p>Антигипертензивные средства. β-адреноблокаторы (особенно неселективные, такие как пропранолол и атенолол) снижают скорость основного обмена (на 4–9%), уменьшают толерантность к физической нагрузке и подавляют липолиз, что приводит к прибавке массы тела в среднем на 1–2 кг [15, 64, 65]. В исследовании GEMINI у пациентов, получавших метопролол, прибавка массы тела составила 0,6 кг за 6 месяцев, тогда как на карведилоле масса тела не изменилась [<xref ref-type="bibr" rid="cit66">66</xref>]. Селективные β-блокаторы с вазодилатирующими свойствами (карведилол, небиволол) имеют более благоприятный метаболический профиль и не ухудшают инсулиночувствительность [<xref ref-type="bibr" rid="cit67">67</xref>].</p><p>Диуретики (гидрохлоротиазид, хлорталидон) и блокаторы кальциевых каналов (амлодипин) считаются нейтральными или ассоциированными с незначительной потерей массы тела (0,4–2,7 кг) [68, 69]. Ингибиторы ангиотензинпревращающего фермента (АПФ) и блокаторы рецепторов ангиотензина II (БРА) не влияют на массу тела или способствуют её умеренному снижению, при этом они улучшают чувствительность к инсулину [70, 71]. Прямые ингибиторы ренина (алискирен) также метаболически нейтральны [<xref ref-type="bibr" rid="cit72">72</xref>].</p><p>Противоэпилептические препараты и другие группы. К ЛИУВТ также предрасполагают противоэпилептические препараты. Вальпроат вызывает прибавку массы тела у 71% пациентов, карбамазепин — у 43%, габапентин и прегабалин также способствуют увеличению массы тела, особенно при длительном применении [73, 74]. Вальпроат ассоциирован с развитием инсулинорезистентности, не зависящей от степени прибавки массы тела, и с высоким риском неалкогольной жировой болезни печени [14, 43]. Топирамат и зонисамид, напротив, ассоциированы с потерей массы тела, что используется в комбинированной терапии ожирения (фентермин/топирамат) [75, 76].</p><p>Среди других групп: антигистаминные средства первого поколения (дифенгидрамин, гидроксизин) могут вызывать прибавку массы тела за счёт блокады H1-рецепторов; протеазные ингибиторы (антиретровирусная терапия) индуцируют липодистрофию и висцеральное ожирение [<xref ref-type="bibr" rid="cit77">77</xref>].</p><p>Клиническая значимость и стратегии ведения пациентов</p><p>Основной проблемой является то, что врачи зачастую откладывают вмешательство до момента, когда индекс массы тела пациента достигает критических значений (ожирение II–III степени). Согласно российским клиническим рекомендациям, диагностика ожирения (код Е66) должна основываться на измерении индекса массы тела (ИМТ) и окружности талии (ОТ) [<xref ref-type="bibr" rid="cit1">1</xref>]. Абдоминальное ожирение (ОТ ≥94 см у мужчин и ≥80 см у женщин) является независимым фактором риска сердечно-сосудистых заболеваний и требует активного вмешательства [1, 2]. В российской популяции сочетание ожирения с артериальной гипертонией и нарушением углеводного обмена статистически значимо ухудшает прогноз [<xref ref-type="bibr" rid="cit2">2</xref>].</p><p>Стратегия управления массой тела на фоне ЛИУВТ должна включать три этапа:</p><p>Принципы пациентоориентированного подхода при лекарственно-индуцированном увеличении массы тела</p><p>Эффективное управление лекарственно-индуцированным увеличением массы тела (ЛИУВТ) невозможно без внедрения принципов пациентоориентированности, которые трансформируют традиционную модель «врач-пациент» в партнерские отношения. Данный подход базируется на следующих ключевых элементах:</p><p>Интеграция этих принципов в клиническую практику позволяет не только эффективнее управлять массой тела, но и укрепляет терапевтический альянс, что является критически важным для лечения хронических заболеваний.</p><p>Заключение</p><p>Лекарственно-индуцированное увеличение массы тела — это неизбежное следствие терапии многих хронических заболеваний, которое требует системного подхода. Игнорирование данного побочного эффекта нивелирует долгосрочные преимущества лечения, способствуя формированию кардиометаболического континуума. Персонализированный выбор препарата с учётом его метаболического профиля, ранний мониторинг (ИМТ, окружность талии, глюкоза, липиды) и мультидисциплинарное взаимодействие (психиатр, эндокринолог, кардиолог) являются основой для минимизации ятрогенных рисков и повышения приверженности пациентов терапии.
