Хронический гепатит С (ХГС) остаётся одной из глобальных проблем здравоохранения, являясь ведущей причиной развития цирроза, гепатоцеллюлярной карциномы и необходимости в трансплантации печени. Сочетание ХГС и ожирения создаёт синергетический негативный эффект, значительно ухудшающий ответ на лечение и прогноз заболевания. В этой связи углублённое изучение патогенетических механизмов, лежащих в основе этой взаимосвязи, в частности роли генетических факторов, представляется крайне востребованным направлением современной гепатологии.</p></sec><sec><title>Цель</title><p>Цель. Изучить влияние полиморфизма rs9939609 гена FTO A23525T на ожирение у больных хроническим гепатитом С.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Исследование проводилось на базе Областной инфекционной клинической больницы города Ярославля, где была сформирована выборка из 240 пациентов в возрасте 18–60 лет. В рамках работы у всех участников были собраны анамнез и выполнены антропометрические измерения (окружность талии, расчёт индекса массы тела). Для верификации хронического гепатита С применяли ИФА, направленный на обнаружение в сыворотке крови спектра антител (a-cor, a-NS3, a-NS4, a-NS-5), а также ПЦР для выявления РНК вируса гепатита С (HCV). Степень активности гепатита оценивали по уровню трансаминаз печени в сыворотке крови. Молекулярно-генетический анализ, включавший исследование полиморфизма rs9939609 гена FTO (A23525T), был проведён в клинико-диагностической лаборатории Института фармации ФГБОУ ВО ЯГМУ. Для генотипирования использовали ПЦР в реальном времени на амплификаторе «iCycler iQ5» (BioRad) с набором реагентов «SNP-экспресс-РВ». Больным проведена эластометрия печени на аппарате FibroScan 502 компании «Echosens» с интерпретацией стадии по шкале METAVIR.</p></sec><sec><title>Результаты</title><p>Результаты. Согласно критериям включения в исследование больные разделены на группы: первая (основная) — ХГС с ожирением (124 человек) и вторая (группа сравнения) — хронический гепатит С без ожирения (116 человек). Установлены корреляционные связи между генотипами гена FTO A23525T и ожирением, а также возможное влияние на степень активности HCV-инфекции и стадии фиброза печени.</p></sec><sec><title>Заключение</title><p>Заключение. У больных ХГС и ожирением установлена прямая корреляционная связь со значением индекса массы тела и окружности талии. При разных генотипах гена FTO установлено возможное влияние на степень активности гепатита и стадии фиброза печени.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. Chronic hepatitis C (CHC) remains one of the global health problems, being the leading cause of cirrhosis, hepatocellular carcinoma, and the need for liver transplantation. The combination of CHC and obesity creates a synergistic negative effect, significantly worsening the response to treatment and the prognosis of the disease. In this regard, an in-depth study of the pathogenetic mechanisms underlying this relationship, particularly the role of genetic factors, is an extremely relevant area of modern hepatology.</p></sec><sec><title>Objective</title><p>Objective. To study the effect of the rs9939609 polymorphism of the FTO A23525T gene on obesity in patients with chronic hepatitis C.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study was conducted at the Regional Infectious Diseases Clinical Hospital in Yaroslavl, where a sample of 240 patients aged 18–60 years was formed. As part of the study, all participants provided their medical histories and underwent anthropometric measurements (waist circumference and body mass index calculation). To verify chronic hepatitis C, we used ELISA to detect a spectrum of antibodies (a-cor, a-NS3, a-NS4, a-NS-5) in the blood serum, as well as PCR to detect hepatitis C virus RNA (HCV). The degree of hepatitis activity was assessed by the level of liver transaminases in the blood serum. Molecular genetic analysis, including the study of the rs9939609 polymorphism of the FTO gene (A23525T), was carried out in the clinical and diagnostic laboratory of the Institute of Pharmacy of the Federal State Budgetary Educational Institution of Higher Education «Yaroslavl State Medical University». Real-time PCR was used for genotyping on the «iCycler iQ5» (BioRad) amplifier with the «SNP-express-RV» reagent kit. The patients underwent liver elastometry using the FibroScan 502 device from Echosens, with interpretation of the METAVIR stage.</p></sec><sec><title>Results</title><p>Results. According to the inclusion criteria, the patients were divided into groups: the first (main) group consisted of patients with CSC and obesity (124 patients), and the second (comparison) group consisted of patients with CSC without obesity (116 patients). Correlations were established between the FTO A23525T gene genotypes and obesity, as well as the possible impact on the severity of HCV infection and the stage of liver fibrosis.</p></sec><sec><title>Conclusion</title><p>Conclusion. In patients with HCV and obesity, there is a direct correlation between the body mass index and waist circumference.
