Изучить влияние гена PPARG на течение гнойных осложнений после лечения травм костей нижних конечностей.</p></sec><sec><title>   Материалы и методы</title><p>   Материалы и методы. Проведено проспективное генетическое исследование с включением 114 пациентов, проходивших лечение в ГБУЗ ЯО «Больница им. Н.В. Соловьева» (Ярославль) в период с 2018 по 2024 гг. по поводу гнойно-воспалительных осложнений после травм нижних конечностей.</p><p>   В качестве объекта исследования выступил полиморфизм гена PPARG C1431T (rs3856806), играющий ключевую роль в регуляции воспалительного ответа и репаративных процессов.</p><p>   Генотипирование выполнено методом ПЦР в реальном времени с последующим рестрикционным анализом. Статистическая обработка данных проводилась с использованием критерия χ² для оценки соответствия распределения генотипов равновесию Харди-Вайнберга.</p></sec><sec><title>   Результаты</title><p>   Результаты. Анализ аллельного распределения гена PPARG выявил преобладание аллеля С в общей выборке (82 % в основной группе и 89 % в контрольной). Наиболее часто встречающимся генотипом оказался С/С (70 и 80 % соответственно), что свидетельствует о его ассоциации с меньшей предрасположенностью к инфекционным осложнениям. Распределение генотипов соответствовало равновесию Харди-Вайнберга (p &gt; 0,05), что подтверждает репрезентативность выборки.</p></sec><sec><title>   Заключение</title><p>   Заключение. Полиморфизм PPARG C1431T является значимым предиктором риска развития и рецидивирования гнойных осложнений у пациентов с травмами нижних конечностей. Носители аллеля Т демонстрируют повышенную вероятность длительного воспалительного ответа, что требует персонифицированного подхода к антибиотикотерапии и ранней иммунокоррекции. Для повышения точности прогноза необходимо учитывать ковариаты, а именно: метаболические нарушения (ИМТ ≥ 30 кг/м², сахарный диабет 2 типа), кардиоваскулярные патологии (гипертоническая болезнь, ишемическая болезнь сердца).</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>   Objective</title><p>   Objective. To study the effect of the PPARG gene on the course of purulent complications after lower limb bone injury treatment.</p></sec><sec><title>   Materials and methods</title><p>   Materials and methods. A prospective genetic study was conducted involving 114 patients who were treated at the N.V. Solovyov Hospital (Yaroslavl) in the period from 2018 to 2024 for purulent-inflammatory complications after lower limb injuries from 2018 to 2024.</p><p>   The polymorphism of the PPARG C1431T gene (rs3856806), which plays a key role in the regulation of the inflammatory response and reparative processes, was used as the object of research.</p><p>   Genotyping was performed using real-time PCR followed by restriction analysis. Statistical data processing was performed using the χ2 criterion to assess the correspondence between genotype distribution and the Hardy–Weinberg equilibrium.</p></sec><sec><title>   Results</title><p>   Results. Analysis of the allelic distribution of the PPARG gene revealed the prevalence of the C allele in the total sample (82% in the main group and 89% in the control group). The most common genotype was C/C (70% and 80%, respectively), which indicates its association with a lower predisposition to infectious complications. The distribution of genotypes was in accordance with the Hardy-Weinberg equilibrium (p &gt; 0.05), confirming the representativeness of the sample.</p></sec><sec><title>   Conclusion</title><p>   Conclusion. The PPARG C1431T polymorphism is a significant predictor of the risk of developing and relapsing purulent complications in patients with lower limb injuries. Carriers of the T allele exhibit an increased likelihood of a prolonged inflammatory response, which requires a personalized approach to antibiotic therapy and early immunocorrection.
