Латентная туберкулёзная инфекция (ЛТИ) представляет глобальную медицинскую проблему, затрагивая 25 % населения мира с ежегодной регистрацией 1,2 миллиона случаев туберкулёза у детей до 15 лет.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Определить особенности соматического статуса у детей с ЛТИ без выявленных контактов с больными туберкулёзом МБТ+.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведено проспективное контролируемое исследование 40 детей в возрасте 8–17 лет (основная группа 20 детей с ЛТИ без контактов с больными туберкулёзом и 20 детей группа сравнения без ЛТИ).</p></sec><sec><title>Результаты</title><p>Результаты. У детей с ЛТИ выявлено снижение фагоцитарной активности лейкоцитов на 30 % относительно нормы, частые респираторные инфекции (89 %), множественные коморбидные нарушения: ожирение 2-й степени (50 % против 10 % в контроле), дисплазия соединительной ткани (40 % против 10 %), аллергические заболевания (60 %), кариес и афтозные стоматиты (34 % против 8,5 %). Высокие титры антител к лямблиям (в 3,2 раза выше контроля) способствуют супрессии иммунного ответа, создавая условия для персистенции M. tuberculosis. Первичное инфицирование МБТ у детей с ЛТИ в 60 % случаев происходило до 3 лет, в контрольной группе — только у 10 %. Несмотря на длительную противотуберкулёзную химиотерапию, положительная реакция на пробу с аллергеном туберкулёзным рекомбинантным сохранялась у 90 % детей с ЛТИ.</p></sec><sec><title>Выводы</title><p>Выводы. Дети с ЛТИ без контактов представляют группу с уникальным иммуно-метаболическим профилем, требующую персонализированного подхода, расширенного календаря прививок и междисциплинарного диспансерного наблюдения не менее 5 лет для снижения рисков развития активного туберкулёза и коррекции коморбидных состояний.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Latent tuberculosis infection (LTBI) represents a global medical problem, affecting 25 % of the world's population with an annual registration of 1.2 million cases of tuberculosis in children under 15 years of age.</p></sec><sec><title>Objective</title><p>Objective. To determine the features of the somatic status in children with LTBI without identified contacts with tuberculosis patients who are MbT+.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A prospective controlled study of 40 children (20 children with LTBI without contacts with tuberculosis patients and 20 without LTBI (control)) aged 8–17 years was conducted.</p></sec><sec><title>Results</title><p>Results. Children with LTBI showed a 30 % decrease in the phagocytic activity of leukocytes relative to the norm, frequent respiratory infections (89 %), and multiple comorbid disorders: grade 2 obesity (50 % versus 10 % in the control), connective tissue dysplasia (40 % versus 10 %), allergic diseases (60 %), dental caries and aphthous stomatitis (34 % versus 8.5 %). High titers of antibodies to Giardia (3.2 times higher than control) contribute to the suppression of the immune response, creating conditions for the persistence of M. tuberculosis. Primary tuberculosis infection in children with LTBI occurred before the age of 3 years in 60 % of cases, whereas in the control group, it occurred only in 10 %. Despite long-term anti-tuberculosis chemotherapy, a positive reaction to the Diaskin test persisted in 90 % of children with LTBI, which is associated with the formation of resident memory T-cells.</p></sec><sec><title>Conclusions</title><p>Conclusions.
